Item Clinico-inflammatory issues of comorbid vibration disease and chronic obstructive pulmonary disease in workers from mechanical engineering(2012) Al-Hayki, Huda; Arkhipkina, Olga; Farid, F.; Fatima, M.Vibration disease (VD) takes one of the leading places in the structure of occupational diseases in the whole world. It often combines with chronic obstructive pulmonary disease (COPD), which also is an actual healthcare problem due to its prevalence, progressive course and mortality. Today one of the pressing problems of medicine is to study the role of cytokines in the pathogenesis of various diseases, because they have a wide spectrum of biological activity and they are regulators of intercellular interactions. Despite the number of studies published concerning VD and COPD, the cytokine profile of their combined course is not yet clear. The purpose of this study was to evaluate the content of cytokines TNF-α and IL-10 in patients with VD and VD, combined with COPD. Materials and Methods. The study involved 118 men who suffered from VD caused by local vibration. The average age of examined patients was 52,3 ± 0,81 years, working experience of contact with the local vibration was 19,13 ± 0,61 years. The main group included 66 patients with COPD and VD. 52 patients with isolated course of VD formed the group comparison. The control group consisted of 20 healthy men. The content of cytokines TNF-α and IL-10 in blood was determined by immunoassay analyzer for Stat Fax 2100 (USA) using reagent kits according to the instructions of the manufacturer "Vektor Best" (Novosibirsk, Russia). Statistical analysis of the results was performed with software Statistica 8.0 for Windows. Results. Determination of the inflammatory cytokine TNF-α showed increasing its concentration in the examined persons. Hereby, the content of TNF-α in patients with VD was significantly higher than in the control group (67,99 ± 4,53 pcg/106 against 42,59 ± 1,98 pcg/106; p<0.05). The level of inflammatory cytokine was more expressed in patients with combined pathology (86,27 ± 4,91 pcg/106) than in patients with isolated course of VD. Decreasing anti-inflammatory cytokine IL-10 occurred in both groups compared with the controls. Level of interleukin in the group comparison was 28,54 ± 2,23 pcg/106 that was lower than in the control group - 33,04 ± 2,51 pcg/106. The maximum decrease of IL-10 observed in the main group - 22,18 ± 1,65 pcg/106. Conclusions. Obtained data showed imbalance in cytokine profile in patients with VD, but more significant changes were found in the examined with accompanying COPD.Item Cytokine profile in patients with both unstable angina and overweight(2013-09) Koteliukh, Mariia; Kovaleva, Yuliia; Shelest, BorysThe purpose include to study the indicators profile of cytokine (TNF-α, IL-1β, IL-4) in patients with both unstable angina and overweight. Materials and Methods: 57 patients were examined, of which unstable angina with obesity installed in 21 patients and unstable angina with overweight in 23 patients. The control group was - 13 people. The study was based in the Kharkiv Clinical hospital № 27. Indicators were studied cytokine profile: TNF-α, IL-1β, IL-4. The immunoassay kit produced by "Ukrmedservis" Ltd. (Ukraine) was used for the investigation of TNF-α, IL-1β, IL-4 parameters, using the manufacturer's instructions. Statistical processing of data was performed using the «Microsoft Exel» 2007 i Windows STATISTIKA 6.0. Results. The level of TNF-α in the patients with both unstable angina and overweight laid within the range 72,13 ± 6,03 pg / ml, significantly (p <0.01) differed from the level of healthy people - 41,4 ± 8,6 pg / ml. The average level for people with both unstable angina associated with obesity was 87,52 ± 7,15 pg / ml that went beyond the control point (p <0.01). The level of IL-1β in the patients with both unstable angina and overweight increased to 162,43 ± 21,71 pg / ml, which was significantly compared with the control - 42,86 ± 7,14 (p <0,01). The level of IL-1β in the patients with both unstable angina and obesity within range 203,85 ± 30,35 pg / ml, significantly increased than in the control group (p<0,01). The level of IL-4 for all patients with both unstable angina and overweight reduced to the range of 9,34 ± 2,11 pg / ml (p<0,01), thus being no different from the control value of 18,25 ± 1,75 (p> 0,1). The level of IL-4 for the patients struggling unstable angina and obesity went down to the range of 4,67 ± 2,05 pg / ml (p <0,05), that is, to the level significantly (p <0, 01) lower than the control value. Conclusions. Thus, activation of inflammation in obesity confirmed scalene change the content of cytokines (TNF-α, IL-1β, IL-4) and reflected the severity of the disease, the mechanism of which remains speculative.Item Dental aspects of cardiometabolic health(2023-03) Assaad, Salma; Melenevych, AnastasiiaCardiometabolic (CM) disorders are directly associated with adverse outcomes such as ischemic heart disease and stroke, which occupy the first two places in the structure of mortality worldwide according to World Health Organization data from December 2020. Recently, a lot of scientific data is emerging regarding the relationship between oral health and CM health. Mutual aggravation is observed – CM disorders, mainly hyperglycemia, insulin resistance, cardiovascular diseases with inadequate control of blood lipids and blood pressure contribute to poor oral health, and oral diseases, in its turn, such as periodontitis aggravate CM pathology by maintaining systemic inflammation.Item Diagnostic and prognostic role of markers of immune inflammation in chronic obstructive pulmonary disease in combination with hypertension(2016) Melenevych, AnastasiiaCurrently 210 million people have chronic obstructive pulmonary disease and it is predicted to become the third leading cause of death worldwide by 2030. In 2008, worldwide, approximately 40% of adults aged 25 and above had been diagnosed with hypertension. We have analyzed the most significant factors associated with increased cardiovascular risk in patients with COPD.Item Item Lung hyperinflation - a prognostically significant marker in geriatric patients(2024-04) Ananya DwivediLung hyperinflation is a common finding in patients with chronic obstructive pulmonary disease (COPD) observed via plain radiograph. Among people older than 65 years, the risk of developing COPD increases five times compared to 40 year olds (Easter M. et al., 2020). In addition, hyperinflation may be found in patients with the Tiffeneau index > 0,7. In most cases, this radiological finding is not given due attention, therefore further diagnosis and treatment is either delayed or insufficiently carried out.Item A modern view on hyperkalemia’s management(2023-10) Melenevych, Anastasiia; Dwivedi, AnanyaThe study demonstrated that sodium polystyrene sulfonate (SPS) and calcium polystyrene sulfonate (CPS) have dominated the management of hyperkalemia over 6 decades after being approved by the United States Food and Drug Administration (FDA) in 1958 because of their long term effi cacy but due to their affi liation with gastrointestinal side effects their usage has been insubstantial or limited. Earlier SPS was thought to work effectively for constipation along with sorbitol but later FDA found it to be causing colonic necrosis and increased mortality. CPS was useful in patients with high sodium load but there were cases with increased vascular calcifi cation. So, eventually in 2009, the FDA prohibited their long-term use for hyperkalemia. The shortcomings led to the development of “new generation” potassium binders including - patiromer and sodium-zirconium cyclosilicate (SZC / ZS-9). In clinical studies patiromer was found to be well tolerated and reduced hyperkalemia in patients with diabetes mellitus, chronic kidney disease, heart failure, hypertension to enhance cardiovascular and renal outcomes and also in patients on renin-angiotensin-aldosterone system inhibitors (RAASi) therapy. Cochrane reviews were conducted three times (2005, 2015, and 2020) for comparison of “old generation” potassium binders with the new generation” potassium binders however, from the point of view of safety, preference was given to the “new generation” potassium binders.Item Particularities of inflammatory markers changes in patients with chronic kidney disease complicated with arterial hypertension(KhNMU, 2013-05) Shelest, Borys; Shatilova, Julia; Kycherenko, DanilThe aim was to study the dynamics of inflammatory markers CRP, IL-1β, IL-4, TNF-α − under the influence of hypertension in patients with CKD. Methods: Included in the study 56 patients with CKD, the first group (n=27) was represented by patients with high blood pressure (BP) systolic blood pressure> 140 mm Hg., BP diastolic <90 mm Hg., the second group - patients with CKD and normal values of BP. The control group consisted of 20 healthy individuals. Cytokines and CRP levels were determined by ELISA. Results and discussion. Revealed a significant increase of inflammatory markers CRP, IL-1β, TNF-α in patients with CKD compared with the control group, whereas anti-inflammatory IL-4 was reduced. For chronic kidney disease with hypertension were characterized by the increasing in CRP, TNF-α and IL-1β and decreasing of anti-inflammatory interleukin-4. It is important to note, that in comparison with the group of patients with normal pressure, patients with hypertension had significantly elevated levels of TNF-α (65,25 ± 4,57, p<0,05). Conclusions. Joining hypertension to CKD leads to greater activation of the inflammatory process in these patients. The level of anti-inflammatory IL-4 was almost equally low in both groups, compared with controls, regardless of the presence of hypertension.Item Practically significant innovations in GOLD 2011 guidelines for diagnosis, management and prevention of chronic obstructive pulmonary disease(2012) Melenevych, Anastasiia; Kalmykov, Oleksiy; Chernyakov, P.; Yasin, Abdul KarimOne of actual practical and scientific problems in general medicine and especially pulmonology is development of chronic obstructive pulmonary disease (COPD). Aim of the investigation was to reveal practically significant innovations in Global Initiative for chronic Obstructive Lung Disease (GOLD) 2011 guidelines for diagnosis, management and prevention of COPD. Material and methods: Comparation of two versions of GOLD guidelines published on official website www.goldcopd.org dated December, 2010 and February, 2012 (last one launched during a symposium hosted by the Asian Pacific Society of Respirology in November 2011) was performed. Only practically significant introductions are represented in this report. Results. The definition of COPD was reworded: 1) “common” - really, during last few years the incidence of COPD increased greatly all over the world; 2) “persistent” (about airflow limitation), while in all previous versions (starting from 2001) more cautious formulation “not fully reversible” was used. Additionally an assured formulation “exacerbations and comorbidities contribute to the overal severity of individual patients” is stated, while previously exacerbations were not included in COPD definition at all, and only presumptive “extrapulmonary effects may contribute” was used instead. Individualization of medical approach to patients with COPD remains in the limelight of COPD definition. Vitally important changes were conducted in diagnosis and assessment of COPD. It is still based on the patient's symptoms, the severity of the spirometric abnormality, identification of comorbidities, while evaluation of future risk of exacerbations was dramatically changed. Significance of lung ventilation function is specially highlighted: “Whereas spirometry was previously used to support a diagnosis of COPD, spirometry is now required to make a confident diagnosis of COPD”. Modernization of COPD management was performed using new diagnosis and assessment approaches to COPD. Strategy considering both disease impact (determined mainly by symptom burden and activity limitation) and future risk of disease progression (especially of the exacerbations) is recommended. Conclusion. New GOLD 2011 guidelines reflect a big step forward made in modern conception of COPD diagnosis, management and prevention. Further perspective from our point of view is to publish comprehensive monographs on the matter under the aegis of GOLD because of perfect analysis methodology provided by this international institution and highest qualification of experts (national leaders from different countries allover the world). This would greatly help medical practitioners and researchers.Item Right ventricular remodeling as an unfavorable feature in patients with chronic obstructive pulmonary disease combined with hypertension(2021-11) Melenevych, Anastasiia; Eshie, VictorWe investigated 69 patients with hypertension (HT) stage II in combination with chronic obstructive pulmonary disease (COPD) (GOLD 2, group B) in remission (57 males and 12 females ; mean age 55,80±5,51 years) who underwent 6-min walk test (6MWT), pulse oximetry, spirometry, chest X-ray, electrocardiography and echocardiography. IL-18 and IL-10 in the serum were determined by ELISA using a test system «Bender MedSystems, GmbH» (Austria). Worsening breathlessness, decreased tolerance to physical activity, reduction in oxygen saturation (SpO2) after 6MWT was associated with RV structural and functional impairment. Significant (p<0,05) differences in right heart chambers, indicating an increase in their overload, was found depending on the presence or absence of desaturation, i.e. decrease SpO2 during 6MWT more than 4% from baseline or below 90%: right atrial size - 39,79±2,27 mm vs. 35,83±5,02 mm; RV diameter - 27,10±2,22 mm vs. 28,37±2,40 mm; RV wall thickness - 5,58±0,57 mm vs. 5,22±0,61 mm; pulmonary artery to aorta ratio - 0,75±0,07 vs. 0,71±0,08. RV pressure overload (RV wall thickness > 5 mm and RV diameter < 30 mm) was associated with an imbalance of inflammatory response with increasing proinflammatory IL-18 and decreasing antiinflammatory IL-10. RV remodeling in response to pressure overload is often accompanied by maladaptive RV structural and functional impairment. Thus, RV remodeling with signs of overload by pressure was associated with increasing breathlessness, decreasing tolerance to physical activity, desaturation during 6MWT and imbalance of inflammatory response in patients with COPD combined with hypertension.Item Role of VEGF in development of chronic obstructive pulmonary disease in combination with hypertension(ХНМУ, 2015-05) Istomina, OlgaNowadays lot of attention is paid to issue of comorbid disease. On the basis of progressive humanity development, it is mentioned a decline of ecological situation, reduction of human motor activity, increasing of bad habits, poor nutrition and organism senilism. All these factors are lead to formation of several disease in humans at the same time. The prevalence of comorbid pathology averages 76,6%, 82% - for women, 72% - for men. One of the most common comorbid conditions is a chronic obstructive pulmonary disease (COPD) in combination with hypertension. According to the data WHO (newsletter #310, 2014) COPD and hypertension are included to the list of ten leading death causes in the world. Hypertension is found in COPD patients with varying frequency (from 6,8% to 76,3%) with average 34,3%. On this stage endothelial dysfunction can be considered as one of the most important independents risk factors for diabetes, atherosclerosis, COPD, hypertension, sepsis and growth of malignant neoplasms. One of the main endothelium functions is vascular growth regulations. One of the angiogenesis markers is vascular endothelial growth factor (VEGF). Among the angiogenic factors family, VEGF is considered to be a major one while neovascularization. Proteins which belong to the VEGF family group are glycoproteins that stimulate the formations of new blood and lymph vessels and also increase vascular permeability. One of the main factors for comobid pathology development is hypoxia and cells structure transformation under the mechanical factors influence that initiates VEGF production. By evaluation of GOLD 2014 revision COPD is characterized by a bronchial tubes affect (obstructive bronchiolitis) so as parenchymal distruction (emphysema). There are an extremely alveolar septa thinning and significantly reducing of their vascularization while progressive capillary endothelium loss and alveolar epithelium that leads to the substitution of lung tissue by the fibrous. Hypoxia promotes HIF-1a stabilization (hypoxia inducible factor-1a) which rapidly degraded at a sufficient oxygen concentration. HIF-1a and HIF-1βb is combined under hypoxia condition. Received complex is binded to a specific sensitive to the hypoxia region of the VEGF genes and increase its expression. Isolated mechanical stretching in a cell culture can contribute to VEGF realizing. Physical activity leading to the increasing of blood flow increasing shear stress and endothelial cells transformation followed by not only by increased VEGF level but also by increasing VEGFRs amount. A growth of shear stress and activation of the renin-angiotensin-aldosterone system while hypertension is also associated with increased VEGF content by mechanical influence on the vascular endothelial cells and angiotensin 2 interaction with angiotensin first type receptors which leads to the expression HIF-1a growth and increasing of VEGF synthesis. Thus VEGF and its receptors can be involved in many processes while COPD and hypertension, such as: airway walls remodeling, endothelial cell apoptosis and vascular remodeling.Item Smoking as modifiable factor in development of cronic obstruktive pulmonary disease in combination with arterial hypertension(ХНМУ, 2015-01-20) Истомина, Ольга Владимировна; Istomina, OlgaChronic obstructive lung pulmonary disease and cardiovascular diaseases have remained the top major causes of mortalitykillers during the past decade. Combination of theseis two pathologiesy aggravates the course of each other,, and represents serious medical and social problems, which leads to health-related quality of life reducing ed quality of life and, eventually, may lead to disability. At the present time, for these diseases can be determine the main modifiable factor that can be determined for these diseases, increasing the risk of them, is such as smoking. Tobacco use is a major cause of many of the world’s top killer mortal diseases illnesses – including cardiovascular diseasepathology, chronic obstructive lung pulmonary disease and lung cancer. In totalgeneral, tobacco use is responsible for the death in of about 1 of ineach 10 adults worldwide. Smokers arise the specific nature of inflammation,s includinges, along with the presence of neutrophils and macrophages, increaseds contents ofin CD8 cytotoxic T-cell and Ncnatural killers1 lymphocytes, presented only in smokers, leading to structural changes in the bronchi. In the later stages, primarily considering growing hypoxia, there are structural changes not only in pulmonary, but also in systemic circulation, but also in the systemic circulation. All this results in the And in the result formed formation of endothelial dysfunctionendothelial dysfunction. It is known, that endothelium regulates the vascular tone. So eEndothelial dysfunction is - an imbalance between substances produced in the endothelium and produced in the endothelium, which are responsible for contraction and expansion dilation of blood vessels. According to the World Health Organization, today 1.3 billion people on our planet is dependent on tobacco. every Every year in Ukraine die from smoking-related causes about 100 000 people die from smoking-related causes, while in Kharkov Kharkiv because of smoking in a year die 3363 people die in a year due to smoking, accounting forabout 9 persons eople daily. So, it could be said, we can say, that smoking is one of the main reasons of COPD in combination with arterial hypertension. And this is a modifiable factor! М.Бронхолегочная патология на фоне дисплазии соединительной ткани отличается особо тяжелым течением, склонностью к хронизации воспалительного процесса, практически необратимыми обструктивными изменениями в легких с клиническими проявлениями бронхоспазма.Item Дисфункція ендотелію і можливості її терапевтичної корекції при хронічному обструктивному захворюванні легенів у поєднанні з артеріальною гіпертензією(2012) Яременко, Вікторія ВасилівнаДисфункція ендотелію — один з головних факторів розвитку і прогресування кардіоваскулярної патології, незалежний предиктор загальної та серцево-судинної смертності. Проте існують лише поодинокі відомості щодо функціонального стану ендотелію при бронхолегеневій патології та її поєднанні з серцево-судинними захворюваннями. Мета: підвищення ефективності діагностики та лікування хронічного обструктивного захворювання легенів (ХОЗЛ) у поєднанні з артеріальною (АГ) гіпертензією на основі вивчення ролі дисфункції ендотелію в патогенетичних механізмах розвитку і прогресування даної патології. Матеріали і методи. Обстеження 110 тематичних пацієнтів та 20 відносно здорових осіб контрольної групи відбуватиметься на базі терапевтичного відділення клініки НДІ гігієни праці та профзахворювань ХНМУ. Додатково до стандартного обстеження проводитиметься дослідження клінічної симптоматики за аналого-візуальними та вербальними шкалами; вивчення якості життя, пов’язаного зі здоров’ям, із використанням опитувальників госпіталя Святого Георгія та SF 36; дослідження біомаркерів функції ендотелію (ендотеліну-1 в сироватці крові імуноферментним методом, метаболітів оксиду азоту в сироватці крові спектрофотометричним методом, визначення ендотелійзалежної вазодилатації та морфофункціональних показників комплексу інтима-медіа ультразвуковим методом); визначення показників центральної гемодинаміки методом ехокардіографії. Висновки. Розв’язання поставлених завдань надасть змогу поглибити уявлення про функціональний стан ендотелію при ХОЗЛ, зокрема на фоні АГ.Item Дослідження рівню біометалів у хворих на вібраційну хворобу у поєднанні з гіпертонічною хворобою(2012) Сухонос, Наталія КостянтинівнаКісткова тканина є гарним провідником та резонатором вібрації, тому ураження опорно-рухового апарату характерні для хворих на вібраційну хворобу (ВХ). Феномен ремоделювання кістки полягає у пристосуванні її механічних властивостей до умов зовнішнього середовища, яке постійно змінюється. Механічний стрес при фізичному навантаженні та наявність супутньої патології є одним з факторів, що впливає на швидкість кісткової перебудови та судинні пошкодження. Матеріали та методи. Обстежено 120 чоловіків, які страждали на ВХ та гіпертонічну хворобу (ГХ), у віці 52,3±0,81 років зі стажем контакту з локальною вібрацією 19,13±0,61 років. Серед них ВХ мали 39 хворих, ВХ з ГХ- 61. Контрольну групу склали 20 практично здорових осіб чоловічої статі. Визначення вмісту біометалів проводили атомно-адсорбційним методом. Статистичну обробку отриманих результатів проводили з використання програмного забезпечення Statistica 8,0 для Windows. Результати досліджень. Вміст кальцію при І ступені ВХ не відрізнявся від групи контролю та становив 2,44±0,01 ммоль/л та 2,38±0,11 ммоль/л відповідно. По мірі наростання важкості захворювання спостерігалося достовірне збільшення рівню біометалу, яке сягало 2,84±0,03 ммоль/л. Зміни неорганічного фосфору також мали місце при ВХ у поєднанні з ГХ порівняно з контролем та характеризувалися його зниженням (0,69±0,11 ммоль/л проти 0,95±0,02 ммоль/л), у той час як при початкових проявах ВХ рівень фосфору відповідав його фізіологічним значенням (0,93±0,05 ммоль/л). Коефіцієнт співвідношення Ca:Р у контролі та у групі хворих з І ступенем ВХ склав 1:0,38, а у при ВХ з ГХ зменшувався до 1:0,24. Дослідження вмісту магнію виявило його зниження, яке прогресувало з посиленням тяжкості ВХ. Рівень Mg у обстежених з І ступенем ВХ склав 0,79±0,02 ммоль/л та 0,61±0,01 ммоль/л у хворих з ГХ проти 1,02±0,04 ммоль/л у контролі. Кальцій/магнієвий коефіцієнт у контролі склав 1:0,43, а обстежених хворих відбувалося його зниження до 1:0,32 при ВХ І ступеня та до 1:0,21 при ВХ з ГХ. Висновки. Отримані результати дають підставу вважати, що у обстежених осіб мають місце зміни мінерального складу вже при І ступені захворювання. Зазначені зміни наростають по мірі прогресування вібраційної патології та при поєднанні з гіпертонічною хворобою. Было обследовано 102 рабочих машиностроения в возрасте 52,3±4,71 лет со стажем работы более 15 лет (20,7±2,05 лет), из профессионлальных групп литейщиков, шлифовщиков, сварщиков. Все группы сопоставимы по демографическим, нозологическим показателям, интенсивности табакокурения. Оценивали клинические показатели, спирографические данные, толерантность к физической нагрузке (ТФН) в тесте 6-минутной ходьбы. Большие частота обострений, темпы прогрессирования заболеваний, выраженность респираторной симптоматики, снижение вентиляционных показателей и ТФН установлены среди сварщиков, в меньшей мере – среди литейщиков и шлифовщиков. Это может быть связано с фиброгенным, токсическим, раздражающим, сенсибилизирующим эффектами сварочного аэрозоля, что обусловливает более выраженные нарушения. Клинические особенности ХБ и ХОЗЛ у больных различных профессиональных групп связаны с условиями труда и прежде всего — физико-химической характеристикой вдыхаемой пыли, что обусловливает существенный профилактический потенциал снижения количества вдыхаемой пыли. Перспективой дальнейших исследований является оценка роли эндогенных (генетических) факторов в развитии данных заболеваний.Item Клініко-функціональні аспекти силікозу у поєднанні з ішемічною хворобою серця(2012) Братусь, Валентина МиколаївнаОднією з найактуальніших проблем внутрішньої медицини є коморбідна патологія, зокрема поєдання хронічних респіраторних захворювань з ішемічною хворобою серця (ІХС). Відомості щодо особливостей перебігу силікозу у поєднанні з ІХС вкрай нечисленні. Обстежено згідно галузевим стандартам 53 чоловіки віком 57,6±5,84 років, середнім «пиловим» стажем у машинобудуванні 24,1±2,5 років, хворих на силікоз I стадії, зокрема, 27 пацієнтів (51 %) з ІХС (стенокардією напруження, I-II ФК, дифузним кардіосклерозом). Пацієнтам з коморбідною патологією властивий атиповий перебіг ІХС (безбольові форми склали 68,2±6,2 %) та більша тяжкість стану (серцеву недостатність встановлено у 82,5±8,1 % випадках, переважно IIA стадії, стенокардію напруження — здебільшого II та III функціональних класів із відповідними електрокардіографічними зрушеннями). Порівняльний аналіз клініко-функціональних показників дозволив встановити у пацієнтів з поєднаною патологією більшу частоту та виразність респіраторної симптоматики (параметрів кашлю та задишки), більш значні зміни функції зовнішнього дихання (як об`ємних, так і швидкісних показників) тощо, ніж у хворих на силікоз без супутньої ІХС. Клініко-функціональні особливості поєднаного перебігу силікозу та ІХС обумовлюють необхідність системного підходу до цієї коморбідної патології з метою патогенетичного обгрунтування ранньої діагностики та вибору оптимальних підходів до її лікування, що становить перспективу подальших досліджень.
