ARDS more often formed against the background of significant blood loss, severe decompensated hemorrhagic shock against the background of trauma, as well as due to pulmonary contusion. Clinically, ARDS manifested by the development of severe acute respiratory failure. All patients necessarily need complex respiratory support. Many scientists studying this problem claim that the risk of developing ARDS increases with the number of blood transfusions. And these experts associate the number of blood transfusions with the risk of developing ARDS. The authors of the publication claim that hemotransfusions are a mandatory component of intensive therapy in a patient with massive blood loss and shock on the background of trauma, without which it is impossible to save the patient. Transfusion of blood and its components have an advantage over the use of plasma exchange solutions against the background of severe shock and lung damage. Therefore, it is incorrect to consider the number of hemotransfusion as an independent predictor of the development of ARDS. Undoubtedly, early motor activation of patients contributes to a better recovery of motor functions and reduces the risk of post-stroke complications. The situation with mor bidity, despite the establishment of quarantine, the introduction of restrictive anti-epidemic measures, and vaccination, remains difficult. The results of research on the influence of meteorological factors on the dynamics of the incidence of COVID-19, hospitalization, and mortality are ambiguous and contradictory. The purpose of this study is to analyze the indicators of morbidity, hospitalization, and mortality from COVID-19 in Ukraine, and to establish the level of influence of meteorological factors on them. A high variation in morbidity, hospitalization, and mortality rates was observed in Ukraine, in 2020‒2021. A total of 3 waves of disease growth were established. The curve of hospitalization indicators of patients with COVID-19 had a correlation dependence on the incidence curve r= 0.766 (р <0.05), the maximum rates of hospitalization and mortality were registered in September–December 2021. A direct strong correlation was established between the frequency of registration of cases of COVID-19 and mortality — r= 0.899 (р <0.05). Most cases of COVID-19 were registered in the cold season, the least in June–August. Inverse correlations of moderate strength were established between the indicators of morbidity, hospitalization, and mortality and air temperature levels (–0.370< r <–0.461). However, studies examining the relationship between COVID-19 and AKI in low- and low-middle income countries (LLMIC) are lacking. Given that AKI is known to carry a higher mortality rate in these countries, it is important to understand differences in this population. Methods: This prospective, observational study examines the AKI incidence and characteristics of 32,210 patients with COVID-19 from 49 countries across all income levels who were admitted to an intensive care unit during their hospital stay. Results: Among patients with COVID-19 admitted to the intensive care unit, AKI incidence was highest in patients in LLMIC, followed by patients in upper-middle income countries (UMIC) and high-income countries (HIC) (53%, 38%, and 30%, respectively), whereas dialysis rates were lowest among patients with AKI from LLMIC and highest among those from HIC (27% vs. 45%). Patients with AKI in LLMIC had the largest proportion of community-acquired AKI (CA-AKI) and highest rate of in-hospital death (79% vs. 54% in HIC and 66% in UMIC). The association between AKI, being from LLMIC and in-hospital death persisted even after adjusting for disease severity. Conclusions: AKI is a particularly devastating complication of COVID-19 among patients from poorer nations where the gaps in accessibility and quality of healthcare delivery have a major impact on patient outcomes.Item Association of Myocardial Changes and Gene Expression of the NFATC1 and NFATC4—Calcineurin Signaling Pathway in Children with Bicuspid Aortic Valve /(2023) Kamenshchyk, Andrii; Gonchar, Margaryta; Oksenych, Valentyn; Kamyshnyi, AleksandrBackground: The role of NFATC gene expression in bicuspid aortic valve (BAV) progression is not fully understood. The aim of this study is to determine the significance of NFATC1 and NFATC4 gene expression for myocardial changes in children with BAV. Methods: In 47 children with BAV, the standard Doppler echocardiographic characteristics were detected, and the expression of the NFATC1 and NFATC4 genes was studied. Results: Posterior wall thickness in diastole (PWTd) and aortic valve peak pressure gradient (AoPPG) in BAV patients were significantly higher compared to healthy controls (PWTd median (min–max), 9 (7–10) mm vs. 7 (6–8) mm; and AoPPG median (min–max), 7.79 (2.98–15.09) mm Hg vs. 2.94 (2.42–3.72) mm Hg). The expression of the NFATC1 gene in BAV children was significantly higher compared to NFATC4 (NFATC1 median (min–max); 70.88 (8.79–106.51) e.u. vs. 7.72 (1.74–22.67) e.u., respectively p < 0.05). A significant correlation of NFATC1 expression with Ao found (R = +0.53, p < 0.05). In BAV patients with PWTd > 8 mm and Ao > 21 mm the NFATC1 expression was significantly higher compared to those with PWTd ≤ 8 mm and Ao ≤ 21 mm (NFATC1 median (min–max); 45.49 (5.01–101.52) e.u. vs. 15.53 (2.36–44.40) e.u., p < 0.05 and 81.11 (20.27–101.10) e.u. mm vs. 12.16 (2.40–45.49) e.u., p < 0.05, respectively). The regulation disruption can be associated with a decrease in insulin production or damage to the insulin signal transduction mechanisms. According to the World Health Organization experts’ assessment, the number of individuals with diabetes worldwide is increasing and has already become a significant medical and social issue for the populations of many countries [2]. This is characterized by the development of micro- and macrovascular complications, leading to the occurrence of heart attacks and strokes. We studied 6,136 breast cancer patients, including 146 with relapses (Group 1) and 455 without relapses (Group 2). We divided the patients based on age, menstrual function, disease stage, histology form and grade, and molecular-biological subtype. The 5-year relapse-free rate for Group 1 was longer for Lum A and TN subtypes (60% and 40%, respectively) but shorter for Lum B and HER-2/neu-amplified subtypes (38% and 31%, respectively). Disease stage, tumor histology, and grade did not significantly affect relapse frequency in these patients. Relapses were more common in premenopausal patients and the Lum B subtype.
