Diagnosis of myocardial contusion in patients with thoracic trauma against the background of polytrauma

Abstract

The authors presented the most common clinical manifestations of myocardial contusion in blunt chest trauma and proposed diagnostic measures that are available for use in most domestic clinics. Clinicians must consider the possibility of traumatic damage to the myocardium in chest injuries, especially if there is a mechanical effect on the front surface of the chest wall. Electrocardiogram monitoring is the most accessible and mandatory diagnostic measure. Changes in the electrocardiogram most often occur in the right chest leads, and manifest as intracardiac conduction disturbances. Central venous pressure in myocardial contusion is often increased, and in case of significant blood loss, the level of central venous pressure is normal. A decrease in cardiac output can be difficult to register with myocardial contusion. Patients with suspected myocardial contusion should receive treatment in the intensive care unit. Electrocardiogram monitoring and oxygen saturation of peripheral arterial blood are mandatory for patients.
