Exact mechanisms of metabolic disorders in hypertension with obesity is still discussible. The aim of the study – to determine the peculiarities of carbohydrate, lipid metabolism changes and activity of adipokines and interleukin-22, in patients with hypertension according to nutritional status. Methods: 80 patients (37 males and 43 females) with essential hypertension (EH) of average age 60.17 years were examined. Carbohydrate, lipid profi les, apolipoprotein B (apo B), tumor necrosis factor-α (TNF-α), plasminogen activator inhibitor-1 (PAI-1), adiponectin, interleukin-22 (IL-22) were estimated. Results: In patients with EH and obesity was found carbohydrates metabolism abnormalities, that was manifested as hyperinsulinemia, glucose and HbA1c levels elevation and insulin resistance (according to HOMA index). Lipid metabolism disorders were observed as valid increasing of triglycerides and apo B. Body mass index elevation was associated with progressive increasing of TNF-α and PAI-1 concentration with reducing of adiponectin level in the patients with EH. Positive relationships between TNF- and HbA1c, apo B; PAI-1 with glucose levels: negative correlation adiponectin with body mass and waist to hip ratio were detected in the patients with obesity associated (BMI ≥ 30 kg/m2) EH. Positive signifi cant correlations between apo B and insulin levels, HOMA index, and TNF-α concentration were defi ned. IL-22 in overweigh and obese patients was signifi cantly higher, correlates negatively with HDL-C. Conclusion: In patients with EH and obesity the adipokine dysfunction was revealed, that correlates with carbohydrate and lipid parameters that indicate increased proinfl ammatory and prothrombogenic processes.Item Diastolic dysfunction, left ventricular and vascular remodeling in hypertensive patients with obesity(2013) Honchar, Oleksii; Гончарь, Олексій Володимирович; Гончарь, Алексей Владимирович; Kovalyova, Olga; Ковальова, Ольга Миколаївна; Ковалёва, Ольга Николаевна; Demydenko, Ganna; Демиденко, Ганна Валеріївна; Демиденко, Анна ВалерьевнаObjective. To investigate the peculiarities of left ventricular (LV) remodeling, diastolic dysfunction (DD) and common carotid artery (CCA) remodeling in hypertensive patients with obesity. Design and method. 75 hypertensive patients (32 male, 43 female) with preserved LV systolic function had been observed, including 51 obese patients (21 male, 30 female). An ultrasound examination of heart (including estimation of transmitral blood flow and mitral valve annulus motion parameters) and CCA was performed. LV geometric pattern, E/A and E/Em ratios, ring segment weight (VM) of CCA were calculated. The statistical analysis was conducted using Mann-Whitney and Pearson χ2 methods. Results. Normal LV geometrical pattern was observed in 6 (11,8%) obese patients and 4 (16,7%) – without obesity, p>0,05; concentric remodeling – 7 (13,7%) and 1 (4,2%), p>0,05; concentric hypertrophy (CH) – 29 (56,9%) and 8 (33,3%), р=0,028; eccentric hypertrophy – (EH) 9 (17,6%) and 10 (41,7%), р=0,015. LV CH prevalence in obese patients was significantly higher compared to EH, p<0,0001. Patients with obesity had higher LVMM (309,3±15,9 vs 258,8±16,8 g, p<0,05), but not MMI (148,5±7,1 vs 139,7±7,9 g/m2, p>0,05). LV DD was revealed in 48 (94,1%) obese patients (including 23 (100%) with II-III st. obesity) and 19 (79,1%) – without obesity, p=0,05. Type I of DD was observed in 31 (60,8%) obese patients, type II – in 17 (33,3%), p=0,028; in non-obese patients – 12 (50,0%) and 7 (29,2%) accordingly, p=0,07. CCA wall hypertrophy was observed in 44 (86,3%) obese patients (including 22 (95,7%) with II-III st. obesity) and 18 (75,0%) – without obesity, p<0,01. The values of mitral valve annulus motion parameters, intimal medial thickness and ring segment weight of CCA are given below as median [LQ; UQ]. Conclusion. Hypertensive patients with obesity were characterized by higher prevalence of LV hypertrophy, concentric patterns of remodeling and DD. Indices of CCA remodeling were also increasing along with BMI. Tissue Doppler parameters of mitral valve annulus motion are important early markers of LV DD.Item Diastolic Dysfunction: from Discovery to the Last Updates(Zaporozhye medical journal., 2019-08) Ashcheulova, Tetyana; Demydenko, Ganna; Gerasimchuk, Nina; Rezunenko, Yuri; Kochubiei, Oksana; Ambrosova, TetyanaOver a long period, chronic heart failure was primarily associated with impaired contractility of the myocardium of the left ventricular, which is a manifestation of systolic dysfunction. Based on modern ideas about the pathophysiology of CHF syndrome, systolic dysfunction is considered as one of the etiological factors along with changes in wall tension and the structure of diastolic filling, i.e. everything that is included in the concept of "LV remodeling". The objective is to present a review of the key parameters of chronic heart failure and left ventricular diastolic dysfunction according to the last updates. Material and methods. The role and significance of diastolic dysfunction as the cause of chronic heart failure syndrome, have only been started to be determined over the past three decades, which can be explained by the difficulty of early diagnostics of diastolic dysfunction. The article describes the historical evolution of estimating heart failure and left ventricular diastolic dysfunction. It is important to note the high prevalence of diastolic dysfunction. The signs of myocardial diastolic dysfunction are detected in people with almost any heart disease. The universal review of the most important parameters which determine this disorder has been presented. It has been shown that nowadays Doppler echocardiography is the noninvasive “golden standard” for estimation of diastolic dysfunction. We have presented methods of treatment for diastolic dysfunction, based on updated recommendations of the American College of Cardiology / American Heart Association in treating patients with heart failure in 2017. Conclusions. The experimental studies show that the survival rate of patients with heart failure with a preserved ejection fraction depends not so much on its indices, but on the severity of diastolic dysfunction. Diastolic dysfunction in patients with heart failure is a more prognostic mortality rate than the ejection fraction, which needs further research for better treatment.Item Dislipidemia, definition, diagnostics and treatment(2017-12) Ashcheulova, Tetyana; Gerasimchuk, Nina; Demydenko, Ganna; Kompaniiets, Kira; Kochubiei, OksanaThe review article presents the pathogenetic role of atherosclerotic vascular lesions in the development of cardiovascular diseases. The relationship between atherosclerosis and inflammation, which is characterized by the identical mechanism in the early phases, which includes the enhancement of the interaction between the vascular endothelium and circulating leukocytes is shown. The definition of such concepts as dyslipidemia, hyperlipoproteinemia and hyperlipidemia is given. The classification of hyperlipoproteinemia by Fredrickson, the clinical classification of dyslipidemia, proposed by the Ukrainian Scientific Society of Cardiologists, 2007 is considered. The correction of dyslipidemia, by both non-medicamentous measures, and drug treatment according to different variants of dyslipidemia is shown. The main groups of lipid-lowering drugs are listed. Their main mechanisms of action to reduce blood lipid levels are noted, and their side effects are listed.
