Item Вплив психоемоційних порушень на параметри товщини інтима-медіа сонних артерій та фактор некрозу пухлин-α у чоловіків молодого віку з артеріальною гіпертензією(2024) Біловол, О.М.; Князькова, I.I.; Kірієнко O.M.; Барбашова, В.О.; Молодан, Д.В.; Абрамова, Л.П.Мета роботи – вивчити особливості змін параметрів структурного стану загальних сонних артерій, а також вмісту фактору некрозу пухлин-α (ФНП-α) в сироватці крові в залежності від психоемоційного статусу у чоловіків молодого віку з артеріальною гіпертензією (АГ). Встановлено, що у чоловіків молодого віку з АГ та психоемоційними порушеннями виявлено достовірно більші рівні систолічного та діастолічного артеріального тиску при офісному вимірюванні та добовому моніторингу артеріального тиску. У молодих людей з АГ 1 ступеня, які систематично не отримували медикаментозну терапію визначено підвищення вмісту ФНП-α в сироватці крові та збільшення товщини комплексу інтима-медіа загальної сонної артерії, більш виражене в осіб з тривожними розладами.Item Зміни вмісту ліпокаліну, асоційованого з желатиназою нейтрофілів, у сироватці крові у пацієнтів молодого віку з артеріальною гіпертензією(2024) Біловол, О.М.; Князькова, I.I.; Kірієнко, O.M.; Барбашова, В.О.; Абрамова, Л.П.У пацієнтів з АГ молодого віку максимальне значення вмісту NGAL у сироватці крові визначено в осіб з АГ та зниженою ШКФ в порівнянні зі здоровими особами та пацієнтами з АГ зі збереженою ШКФ. За даними дуплексного сканування ниркових артерій встановлено, що у пацієнтів з АГ молодого віку реєструвалось зниження кінцевої діастолічної швидкості артеріального кровотоку (Ved) і підвищення індексу резистентності (RI), більш виражені при зниженні ШКФ, що підтверджує їх діагностичну значущість при оцінці морфо-функціональних змін нирок.Item Daily blood pressure parameters and structural and functional changes in the myocardium of patients with hypertension and frequent ventricular extrasystole(2024) Ivankova, A.V.; Kuzminova, N.V.; Lozinsky, S.E.; Kniazkova, I.I.; Ivanova, E.I.; Shkarivsky, Yu.L.; Romanova, V.O.We examined 50 patients with stage II hypertension and frequent symptomatic ventricular extrasystoles and 32 patients with stage II hypertension without any cardiac arrhythmias. All patients underwent a comprehensive clinical and instrumental examination, daily blood pressure monitoring, and echocardiographic examination. It was determined that patients with ventricular extrasystoles had significantly (p<0.04) higher systolic blood pressure values during the day and at night. Systolic blood pressure variability at night showed the highest values in patients with frequent extrasystoles, which may indicate a complicated course of hypertension in this category of patients and increase the risk of target organ damage. In addition, frequent ventricular extrasystole in patients with stage II hypertension was associated with right heart overload, with an increase in the frequency of mitral valve fibrosis/calcification and tricuspid regurgitation, a decrease in ejection fraction and a deterioration in myocardial relaxation capacity.Item Changes of the cardiovascular system state in young men depending on their psychological status(2024) Bilovol, O.M.; Kniazkova, I.I.; Mishchenko, V.M.; Golovachova, V.O.; Kuzminova, N.V.; Kryvoshapka, О.V.; Abramova, L.P.The purpose of the study was to establish the influence of the psycho-emotional state on clinical and hemodynamic parameters and the level of physical performance in young men. 82 young men (average age 28.4±5.2 years) without cardiovascular pathology and any somatic diseases in the anamnesis were examined. Depending on the psycho-emotional state, according to the anxiety questionnaire by Ch. D. Spielberg, the examined white men were divided into 2 groups: the main – with high anxiety – 43 people and the control – 39 people without violations of psycho-emotional status. It was established that young men without pathology of the cardiovascular system with a high level of anxiety according to the scale of Ch. D. Spielberg had reduced physical capacity according to the test with dosed physical load. It was determined that young men with reduced physical capacity and increased anxiety have a high prevalence of heart rhythm disorders according to daily ECG monitoring and signs of left ventricular diastolic dysfunction.Item Prognostic significance of non-specific systemic inflammation markers in patients with coronary heart disease(2024) Romanova, V.O.; Kuzminova, N.V.; Ivankova, A.V.; Lozinsky, S.E.; Kniazkova, I.I.; Poberezhets, O.L.; Repetenko, M.V.To evaluate the prognostic significance of markers of nonspecific systemic inflammation as predictors of unfavorable prognosis in patients with coronary heart disease, data from 135 patients with uncomplicated disease and 27 with unfavorable outcomes were analyzed. A 2-year prospective study showed that unfavorable cardiovascular events were significantly more common in patients with coronary heart disease and elevated levels of biomarkers of nonspecific systemic inflammation compared with those with lower values. The presence of myocardial infarction at baseline, a decrease in left ventricular ejection fraction <50 %, and increased levels of markers of nonspecific systemic inflammation such as C-reactive protein, determined by a highly sensitive method, and tumor necrosis factor-α had significant predictive value for the likelihood of unfavorable events.Item The influence of psychoemotional disorders on the thickness parameters of the intima-media of carotid arteries and tumor necrosis factor-α in young men with arterial hypertension(2024) Bilovol, O.M.; Kniazkova, I.I.; Kіrienko, O.M.; Kuzminova, N.V.; Mishchenko, V.K.; Kіrienko, D.O.; Abramova, L.P.The purpose of the study was to establish the peculiarities of changes in the parameters of the structural state of the common carotid arteries, as well as the content of tumor necrosis factor-α in blood serum depending on psycho-emotional status in young men with arterial hypertension. We examined 76 men (mean age 36.4±3.5 years) with grade 1 hypertension, including 55 patients with moderate and high anxiety (group 1) and no disorders of psychoemotional status (group 2). The control group consisted of 20 healthy men. It was established that in group 1, the levels of systolic and diastolic blood pressure during office measurement and daily monitoring exceeded (p<0.05) the data of group 2. Also, in group 1, the serum levels of tumor necrosis factor-α and the thickness of the intima-media of the carotid arteries were higher (p<0.05) than the indicators of group 2. Thus, in young patients with arterial hypertension, there is an increase in the serum content of tumor necrosis factor-α and signs of structural changes in the carotid arteries, more pronounced in the group with anxiety disorders.Item Natriuretic peptide: diagnostic, profnostic, and clinical relevance in patients with type 2 diabetes mellitus, arterial hypertension and obesity(2024-01-01) Dunaieva, InnaNatriuretic peptides are one of the main biomarkers of chronic heart failure (CHF). The level of NT-proBNP makes it possible to evaluate the negative prognosis in patients with CHF, as well as to monitor the sensitivity of patients with left ventricular dysfunction to the appointment of treatment. The purpose of the study: to study the NT-proBNP biomarker in comorbid patients with hypertension, obesity, type 2 diabetes and determine the relationship with a number of hormonal and biochemical indicators. Materials and methods: all patients were divided into 4 groups depending on the presence of comorbid pathology in them: patients with hypertension — 1 group — 22 people; patients with hypertension combined with obesity — group 2 — 30 people; Hypertension in combination with type 2 diabetes — group 3 — 31 people; patients with hypertension, type 2 diabetes and obesity — group 4 — 28 people. Determination of the content of N-terminal brain natriuretic peptide (NT-proBNP) in the blood serum of patients was carried out by the immu-noenzymatic method on the analyzer «Labline-90» (Austria) using a commercial test system manufactured by the company «Elabscience» (ELISA, China) according to the instructions, which was part of the set. Conclusions. It was established that in patients with hypertension, hypertension in combination with obesity, hypertension in combination with type 2 diabetes, as well as hypertension in combination with type 2 diabetes and obesity, there is a significant increase in NT-proBNP compared to the indicator of the control group (p < 0.05). It has been proven that NT-proBNP has a direct correlation with creatinine (p = 0.045), urea (p = 0.038), triglyceride level (p = 0.027) and cholesterol (p = 0.031); direct correlation with the level of HbA1c (r = 0.202; p = 0.0038). The level of NT-proBNP has a significant predictive value for the risk of developing car-diovascular complications in patients with hypertension.Item Cystatin C improves cardiovascular risk prediction in cardiometabolic patients in addition to estimated glomerular filtration rate(Via Medica, 2024-03-13) Dunaieva, InnaItem Apelin-13 level, diastolic dysfunction, and cardiac remodeling in patients with arterial hypertension and extrasystole(2024) Ivankova, A.V.; Kuzminova, N.V.; Lozinsky, S.E.; Knyazkova, I.I.; Kulchytska, O.M.; Poberezhets, O.L.; Romanova, V.O.A total of 156 patients with stage II hypertension were examined, of whom 124 had frequent extrasystoles and 32 did not have any heart rhythm disorders. All patients included in the study underwent a complete clinical, laboratory, and instrumental examination, including measurement of serum apelin- 13, determination of diastolic dysfunction, and type of structural and geometric cardiac remodeling. The presence of frequent extrasystoles (both supraventricular and ventricular origin) in patients with stage II hypertension was associated with the presence of diastolic dysfunction, namely, with a deterioration in myocardial relaxation capacity. A significant inverse relationship between the level of serum apelin-13 and the presence of frequent extrasystoles, in particular of ventricular origin, male gender, smoking, and hereditary burden in patients with hypertension was established.Item The content of neutrophil gelatinase-associated lipocalin in the blood serum of young patients with arterial hypertension(2024) Bilovol, O.M.; Kniazkova, I.I.; Mishchenko, V.M.; Kuzminova, N.V.; Kirienko, O.M.; Abolmasov, O.M.; Abramova, L.PThe purpose of the work was to study the content of neutrophil gelatinase associated lipocalin in blood serum and its relationship with parameters of intrarenal hemodynamics in young patients with arterial hypertension. It was established that in such patients the maximum value of neutrophil gelatinase associated lipocalin content in blood serum was characteristic of individuals with a reduced glomerular filtration speed compared to patients with preserved glomerular filtration speed and healthy individuals. The duplex scan data of the renal arteries showed that in patients with arterial hypertension of young age a decrease in the end-diastolic velocity of arterial blood flow and increasing the resistance index was recorded, more pronounced when the rate of glomerular filtration decreases, that confirms their diagnostic significance in the assessment of morpho-functional changes in the kidneys.
