Familial Mediterranean fever (FMF) is a non-infectious genetic disease caused by a mutation in the MEFV gene. This disease is more common in people of Mediterranean or Middle Eastern descent, but can occur in any ethnic group, including those living European countries. FMF has typical clinical symptoms. Its long term without appropriate treatment the disease can lead to a violation of the child's physical development and result in the development of amyloidosis. The aim is to focus the attention of the medical community of Ukraine and inform European medical specialists on the FMF cases due to migration changes in Ukraine. This article presents a clinical case of FMF in a 4-year-old child who was admitted to our hospital with complaints of fever, abdominal pain, vomiting, and stool retention. Additional examination included: laboratory tests (complete blood count, urinalysis, biochemical blood test with determination of liver and kidney tests); instrumental methods: ultrasound examination of the abdominal organs and urinary system, X-ray examination of the abdominal organs with contrast. The results of additional tests revealed the signs of inflammatory process and impaired kidney function. When clarifying the family medical history, it was revealed that the older brother suffers from FMF (a mutation of the MEFV genotype was determined) and takes specific therapy (colchicine). The final diagnosis was determined: Acute strangulated intestinal obstruction Familial Mediterranean Fever (clinically). Peritonitis. Operative laparoscopic and conservative treatment was performed. The patient was discharged in a satisfactory condition with appropriate recommendations. Conclusions. The number of FMF cases in children have increased due to changes in the migration process in Ukraine. The relevance of this disease has increased. Taking into consideration a significant migration of population, the likelihood of an increase in the number of cases of FMF in European countries is increasing, which requires awareness among a wide range of doctors. In this report we have described a pediatric FMF case which was represented by recurrent episodes of fever, abdominal pain, impaired kidney function test results, and inflammatory markers. A determined mutation of the MEFV genotype in the brother of our patient makes the diagnosis of FMF unquestionable. The research was carried out in accordance with the principles of the Helsinki Declaration. The informed consent of the patient was obtained for conducting the studies. The relationship between GERD and BA is complicated and controversial. Aim: to analyze impact factors of BA and GERD in children with comorbid pathology for the further development of early diagnostic and preventive measures. Materials and methods. 81 children (aged 6 to 18 years) were involved in the study and divided into two groups: children with GERD and BA as a main group (n=27) and children with GERD and without allergy as a control group (n=54). The verification of diagnoses was carried out according to the unified clinical protocols of medical care. The results were processed using nonparametric statistical methods. Results. Esophageal motility disorders (EM) were detected in 100% of patients with BA, regardless of the severity. Inflammatory changes of the esophageal mucosa (EM) were equally frequently detected in patients with mild and moderate persistent BA, and were absent in patients with severe BA. GERD was diagnosed in 44.4% of patients with BA. 25.0% of patients had the erosive form of GERD, 25.0% — non-erosive GERD, 50.0% — endoscopically negative GERD, in the control group — 33.0%, 35.0%, 32.0% respectively. Conclusion. All patients with BA had esophageal motility disorders. GERD was diagnosed in 44.4%. Inflammatory changes of EM (erosive and non-erosive forms of GERD) were detected in 22.0% of patients with mild and moderate forms of BA, and in severe forms of BA — endoscopically negative GERD. The frequency of inflammatory changes reached 68.0% in the control group. The research was carried out in accordance with the principles of the Declaration of Helsinki. The study protocol was approved by the Local Ethics Committee of the institution indicated in the work. The informed consent of children's parents was obtained for conducting the studies. No conflict of interests was declared by the authors.Item Impact of Environmental Factors on the Level of Physical Growth and Development of Children in Kharkiv Region. Children living in the ecologically unfavorable regions were found to have the linear growth showings increment, and the increment degree of this index depends on the incorporation rate of conditionally-toxic micro-elements (r = 0.72). Gender peculiarities of ecological factors influence on the body weight of children index were found. Thus, in boys living in the ecologically unfavorable regions body weight measures are lower than age-related norms, while in girls decrease of these measures is found in ecologically favorable regions. Environmental degradation leads to disbalance and physical development periodicity disbalance, which has a negative effect on the health condition in general. У роботі наведені результати дослідження рівня фізичного розвитку 2600 дітей, які мешкають у різних екологічних умовах Харківського регіону. Встановлено, що у дітей з несприятливо екологічних районів відбувається прискорення лінійного зросту, а ступінь прибавки цього показника у дитячому віці має кореляційну залежність від ступеню накопичення умовно-токсичних мікроелементів (r=0,72). Виявлені певні гендерні особливості впливу екологічних факторів на показники маси тіла дітей. Так, у хлопчиків мешканців екологічно несприятливих районів маса тіла нижче вікових нормативів, тоді як у дівчаток нижчі показники маси тіла спостерігаються у мешканців екологічно сприятливих районів. The express algorithm of hygienic research to assess the general hazard of chemical compounds to human health allows targeted hygienic research to study HF depending on the goals and objectives. Materials and methods: At the 1 stage, an analysis of the PD frequency, the pregnancy, the labor was conducted during 5 years. At the 2 stage, 188 mother-child pairs were examined: 84.04% women had PD and 15.96% didn’t have it. The research included the analysis of the anamnestic data, maternal nutritional status, general clinical study, assessment of the physical, psychomotor level of the child’s development, study of the elemental profile. Results: High frequency of complications in pregnancy and labor was observed in cases when women had PD, due to the imbalance in the “mother-placenta-fetus” system. The results’ analysis showed an increased level of Zn (1.437%), K (10.147%), and Ca (83.900%) in hair; an increased level of K (82.818%), Cr (0.274%), and Na (3.611%) in breast milk of women with PD. Children born to mothers with PD had a significantly increased level of Cr (0.92%), S (0.578%) and P (0.169%), Na (0.107%), Ca (56.041%), and Zn (7.149%). Метою дослідження було аналіз вітамінно-мінеральної забезпеченості дітей шкільного віку. Комплексно із залучанням різних за фахом спеціалістів обстежено 230 дітей віком 7-17 років. Оцінку вмісту вітамінів у сироватці крові проводили з урахуванням аналізу фактичного харчування методом високоефективної рідинної хроматографії на апараті «Varian». Визначення вмісту макро- та мікроелементів (МЕ) у волоссі дітей методом характеристичного рентгенівського випромінювання на рентгенівському спектрометрі “X-Lab 2000”. Аналіз результатів дослідження показав, що харчування дітей шкільного віку на 30-40% має енергетичний дефіцит, який пов’язаний з недостатністю білка та надмірним споживанням вуглеводів (понад 20,0%) за рахунок простих сахарів. У всіх обстежених школярів у раціоні відмічається виразна недостатність вітамінів А, групи В, Е, Д та вітаміну С. Дослідження вмісту вітамінів у сироватці крові показали, що усі діти мають дефіцит хоча б одного з головних вітамінів, а 73,8% дітей мають полівітаміну недостатність. Виявлено, що 78,8% дітей мають мінеральні порушення та дисбаланс співвідношення головних ессенціальних елементів. Встановлено, що у 66,7% дітей з встановленим гіповітамінозом та дисбалансом мінеральних речовин в анамнезі мали місце довготривалий період одужання після перенесених гострих захворювань та 23,1% дітей мали різноманітні ускладнення після цих захворювань. Минеральный дисбаланс и гиповитаминозы оказывают существенное влияние на уровень здоровья ребенка. Цель исследования: анализ обеспеченности витаминами и минеральными веществами детей школьного возраста. Комплексно, совместно со специалистами, обследовано 230 детей 7-17 лет. Оценку уровня витаминов в сыворотке крови проводили с учетом фактического питания методом высокоэффективной жидкостной хроматографии на аппарате «Varian». Определение содержания макро- и микроэлементов (МЭ) в волосах детей проводили методом характеристического рентгеновского излучения на рентгеновском спектрометре “X-Lab 2000”. Анализ результатов исследования показал, что рацион детей школьного возраста имеет энергетический дефицит на 30-40%, который связан с недостаточностью белка в рационе на фоне повышенного употребления углеводов (более 20%) за счет простых сахаров. У всех обследованных школьников в рацион дефицитен по витамину А, группы В, Е, Д и витамину С. Данные исследования уровней витаминов в сыворотке крови показали, что все дети имеют дефицит хотя бы одного из основных витаминов, а 73,8% детей имеют поливитаминную недостаточность. Установлено, что 78,8% детей имеют минеральные нарушения и дисбаланс соотношения основных эссенциальных элементов. Установлено, что у 66,7% детей с выявленным витаминно-минеральным дисбалансом в анамнезе отмечали длительный период выздоровления острых заболеваний и 23,1% детей имели осложнения после этих заболеваний. The mineral imbalance and hypovitaminosis significantly affect the level of child health. The aim of the study was the analysis of vitamin and mineral supply of school children. 230 children aged 7-17 years underwent comprehensive examination with involvement of different specialty experts. Determination of vitamin content was conducted by high performance liquid chromatography on apparatus «Varian». Determination of macro- and microelements (ME) content in hair of children was performed by characteristic X-rays method on X-ray spectrometer "X-Lab 2000". Analysis of the results of the study showed that nutrition of school children has 30-40% energy deficit, which is associated with protein deficiency and excessive intake of carbohydrates (over 20.0%) due to simple sugars. All studies students in their diet showed a distinct deficiency of vitamins A, B, E, D, and vitamin C. It is established that all children have deficiency of at least one of the major vitamins, and 73.8% of children had multivitamins failure, It was revealed that 78.8% of children had mineral imbalance of ratio of essential key elements. Полученные результаты свидетельствуют, что развитие или обострение хронического заболевания почек у детей может быть обусловлено неблагоприятным преморбидным фоном. Обострению почечного заболевания у некоторых детей способствовали вирусные и бактериальные инфекции, но у некоторых этиологический фактор обострения патологического почечного процесса выявить не удалось. Встановлено, що у дітей без ознак дизморфогенезу сполучної тканини найбільш виразний дисбаланс відмічається в групі мікроелементів які приймають участь в процесах кісткоутворення із суттєвим зменшенням показників кальцію. У дітей з порушенням фібрилогенезу мінеральні зсуви відбуваються насамперед за рахунок дисбалансу ессенціальних мікроелементів: кальцію, магнію, цинку та бору.
