According to the Guidetothe Development of Children's Palliative Care Services life-limiting conditions may be delineated at four broad groups. Cystic fibrosis is a problem from group 2 - conditions where there may be long periods of intensive treatment aimed at prolonging life and allowing participation in normal childhood activities, but premature death is still possible. Basic aspects of CFRD were discussed, such as cause of the disease, genetic defects, differential diagnosis and principles of treatment. Two clinical cases of pediatric CFRD patients resented to highlight problems of management in terms of potential palliative care. According to our observations, patients with the CFRD, unfortunately, are perceived as patients with cystic fibrosis with no clear attention to diabetes. Both of them were presented with pure diabetes control due to unclear understanding CFRD with concomitant diet incompliance, irregular records, missed insulin injections, exercising limitations. We concluded that CFRD patients must be assessed by members of multidisciplinary team at CF centers, working by the principle of palliative care. There is necessary since the moment of CF diagnosing to involve endocrinologist and dietician due to of endocrine problems. The younger the child, the more often these diseases occur and the more severe it can be. Cardiovascular disorders with bronchopulmonary pathology develop slowly, potentially reversible in childhood, which requires a correct assessment of the risk of their development for a particular patient and timely correction of therapy. The presence of dyspnea, prolonged exhalation, pale skin, cyanosis of the nasolabial triangle and acrocyanosis, swelling of the nose (symptoms common to cardiac and respiratory failure) indicate the reaction of the cardiovascular system in diseases of the respiratory system. We examined 14 children: among them 7 boys and 7 girls. Ten children were diagnosed with acute obstructive bronchitis, and 4 with pneumonia. In the course of the study, the parameters were assessed, reflecting the linear dimensions of the heart cavity and the main vessels, followed by the calculation of central hemodynamics, diastolic transmittal blood flow by the method of L.K. Angelsen.Item Nutritional rickets in children (case report)(2019) Chaychenko, Tetyana; Shulha, N.; Kharkovа, M.; Rybka, O.; Buzhinska, Nadiya; Chercun, A.; Чайченко, Т.В; Рибка, О.С.; Харькова, М.О.; Бужинська, Н.Р.Nutritional rickets (NR) caused by vitamin D and/or dietary calcium deficiency remains a significant global, public health problem. The main causes of NR are discussed with a focus on the investigations, management and prophylaxis by the Global Consensus Recommendations on Prevention and Management of NR (2016). The article presents a clinical case of NR in a girl aged 4 years and 3 months admitted to our hospital with severe deformation of low extremities. She was found to have vitamin D-deficient state with extremely high alkaline phosphatase, suggestive for massive bone resorption. NR was diagnosed following comprehensive examination and therapy according to the protocol was initiated. Is funded some works of acting medicines on syntheses monophosphats by leucocytes. Purpose. Improving the efficiency of the prognosis of development bronchitis obliterates by analysis c-AMF/ c-GMF in sputum’s leucocytes. Patients and methods. Electrophoresis conducted with ELISA to determine the performance of c-AMF/ c-GMF serum in 72 children with bronchitis obliterates. Results. Found that children with a bronchitis obliterates characterized by reduced levels c-AMF (19,7 [15,5; 25,6] pmol/ml) and increased levels c-GMF (21,6 [16,2; 27,9] pmol/ml), which is an indicator of prolong fibrosis of bronchioles. c-AMF/ c-GMF was increase (1,02±0,127 U). High level of c-GMF is an early marker of interstitial prediction of a pure prognosis and lung hypertension (Wilks' lambda - 0,926 F(1,19)=15,49; р=0,0001). Conclusion.
