Exact mechanisms of metabolic disorders in hypertension with obesity is still discussible. The aim of the study – to determine the peculiarities of carbohydrate, lipid metabolism changes and activity of adipokines and interleukin-22, in patients with hypertension according to nutritional status. Methods: 80 patients (37 males and 43 females) with essential hypertension (EH) of average age 60.17 years were examined. Carbohydrate, lipid profi les, apolipoprotein B (apo B), tumor necrosis factor-α (TNF-α), plasminogen activator inhibitor-1 (PAI-1), adiponectin, interleukin-22 (IL-22) were estimated. Results: In patients with EH and obesity was found carbohydrates metabolism abnormalities, that was manifested as hyperinsulinemia, glucose and HbA1c levels elevation and insulin resistance (according to HOMA index). Lipid metabolism disorders were observed as valid increasing of triglycerides and apo B. Body mass index elevation was associated with progressive increasing of TNF-α and PAI-1 concentration with reducing of adiponectin level in the patients with EH. Positive relationships between TNF- and HbA1c, apo B; PAI-1 with glucose levels: negative correlation adiponectin with body mass and waist to hip ratio were detected in the patients with obesity associated (BMI ≥ 30 kg/m2) EH. Positive signifi cant correlations between apo B and insulin levels, HOMA index, and TNF-α concentration were defi ned. IL-22 in overweigh and obese patients was signifi cantly higher, correlates negatively with HDL-C. They are connected with adipokines in the regulatory actions. The aim of the article was to investigate the role of the main parts of RANK-RANKL-osteoprotegerin axis and cytokines of additional link of regulatory network – adiponectin and visfatin and the interrelationship of these cytokines in chronic kidney disorder model in rats. Chronic kidney disorder model was created in the rats 12 weeks after intramuscular 50 % glycerol injection-induced acute kidney injury. RANKL, osteoprotegerin, adiponectin and visfatin levels was measured using ELISA. Increase of all cytokines and changes of their interrelations has been revealed in rats with chronic kidney disorder model. У таких больных поражение органов-мишеней обусловлено прогрессированием метаболических нарушений в печени, инсулинорезистентностью, ожирением, нарушениями липидного и углеводного обменов, развитием системного воспаления и субклинических признаков атеросклероза, которые находятся во взаимосвязи с дисбалансом адипоцитокинов и избыточным накоплением висцеральной жировой ткани, что способствует развитию высокого кардиометаболического риска у данной категории больных. В роботі представлено результати аналізу факторів кардіометаболічного ризику у пацієнтів з неалкогольною жировою хворобою печінки і метаболічним синдромом. У таких хворих ураження органів-мішеней обумовлено прогресуванням метаболічних порушені в печінці, інсулінорезистентністю, ожирінням, порушеннями ліпідного і вуглеводного обмінів, розвитком системного запалення та субклінічних ознак атеросклерозу, які знаходяться у взаємозв’язку з дисбалансом адипоцитокінів і надлишковим накопиченням вісцеральної жирової тканини, що сприяє розвитку високого кардіометаболічного ризику у даної категорії хворих. Article presents the result of evaluation of cardiometabolic risk factors in patients with nonalocoholic fatty liver disease and metabolic syndrome. The organs damage in this patients caused by the progression of metabolic disorders in the liver, insulin resistance, obesity, lipid and carbohydrate metabolism and the development of systemic inflammation and subclinical atherosclerosis who correlate with the imbalance of adipocytokines and excessive accumulation of visceral adipose tissue.Item Фактори формування метаболічного синдрому у пацієнтів з неалкогольною жировою хворобою печінки(2013) Боброннікова, Леся РоманівнаВ статье представлены результаты исследований 45 пациентов с неалкогольной жировой болезнью печени с нормальной массой тела и абдоминальным ожирением. Установлено, что течение НАЖБП и АО сопровождается ранним формированием метаболического синдрома, обусловленного дислипидемией, гиперинсулинемией, инсулинорезистентностью и развитием системного воспаления, а также дисбалансом адипоцитокинов, характеризующемуся повышенным уровнем лептина, фактора некроза опухолей-α и сниженным уровнем адипонектина. В статті представлені результати обстеження 45 пацієнтів з неалкогольною жировою хворобою печінки з нормальною масою тіла та абдомінальним ожирінням. Встановлено, що перебіг неалкогольної жирової хвороби печінки та абдомінального ожиріння супроводжується раннім формуванням метаболічного синдрому, обумовленого дисліпідемією, гіперінсулінемією, інсулінорезистентністю і розвитком системного запалення, а також дисбалансом адипоцитокінів, що характеризується підвищеним рівнем лептину, фактору некрозу пухлин-α і зниженим рівнем адипонектину. The article presents the results of examination of 45 patients with nonalcoholic fatty liver disease with normal body mass and abdominal obesity. It is set, that the nonalcoholic fatty liver disease and abdominal obesity accompanied the early formation of the metabolic syndrome caused dyslipidemia, hyperinsulinemia, insulin resistance and the development of systemic inflammation, as well as an imbalance of adipocytokines, characterized by elevated levels of leptin, TNF-α and decreased levels of adiponectin.
