Abstract: Comorbid pathology, e.g. combination of chronic respiratory diseases with coronary heart disease (CHD) is one of most actual problem of internal medicine, while data about course and signs of silicosis accompanied by CHD is extremely few. Aims and objectives: To find out whether silicosis combined with ischemic heart disease has any clinical peculiarities. 53 males with 1st stage of silicosis, including 27 patients with CHD (stenocardia of effort, 1st-2nd functional class, diffuse cardiosclerosis) aged 57,6±5,84 years with mean work experience in machine-building industry of 24,1±2,5 were examined according to conventional standards. Special attention was paid to type of course, characteristics of clinical signs related to respiratory or cardiovascular pathology. Patients with comorbid pathology was characterized by atypic course of CHD (painless forms was stated in 68,2±6,2 %) and higher severity of clinical state (e.g., heart failure was revealed in 82,5±8,1 % cases, mostly of 2nd A stage, angina pectoris — of mostly 2nd and 3rd functional classes with appropriate electrocardiographic changes). Comparative analysis of clinical manifestation made possible to state higher frequency and intensity of clinical signs (cough, dyspnea, pain parameters), more significant lung ventilation disturbances in patients with combined pathology opposite to patients with silicosis along. Clinical peculiarities of silicosis accompanied by CHD specify the importance of systemic approach to them aimed on pathogenetic grounding of early diagnosis and chose of optimal approaches to the treatment of this comorbid pathology, this forms the perspective of further research.
