Abstract: У роботі наведені патогенетичні особливості формування остепенічих порушень у дітей з соматичною патологією. Так, у дітей з патологією травної системи остеопенія діагностується у 56,6% випадків, у 59,8% дітей з патологією нирок та у 51,2% дітей з бронхолегеневою патологією. Важкість остеопенії у всіх групах пацієнтів залежить від тривалості захворювання, частоти загострень та важкості перебігу основного захворювання. Аналіз показників кісткового моделювання показав, що для дітей з патологією травної системи притаманне прискорення кісткової резорбції, для дітей з патологією нирок - порушення метаболізму кісткової тканини, а саме мінералізації кісток та для дітей з бронхолегеневою патологією - гальмуванні процесів кіскоутворення. Таким чином, патогенетичні аспекти формування порушень кісткової тканини при різних патологічних станах має певні особливості, що необхідно враховувати при ведені таких пацієнтів.Study of features of structural and functional state disorders of the bone tissue in children with chronic somatic pathology is one of the important directions of modern pediatrics. The purpose of the study was to reveal the pathogenetic mechanisms of osteopenic disorders in children with chronic somatic diseases. Materials and methods of the study. 202 patients with chronic kidney diseases (59 children), gastrointestinal diseases (80 children) and bronchopulmonary pathologies (63 children) were examined. The results of ultrasound densitometry ("Sonos - 2000") and biochemical studies were analyzed with help of clinical and information processing methods and international bioethical standards. Results and their discussion. In 56.6% of children with gastrointestinal diseases osteopenia was diagnosed. The severity of osteopenia depends on the frequency of exacerbations (r = 0.89), duration of the main disease (r = 0.68) and the inflammation of the colon (r = 0.65). Analysis of the biochemical markers of bone tissue showed the reduction of ionized calcium on 12.0%, reduction of bone isoenzyme of alkaline phosphatase and keratin sulfate, as well as increased excretion of hydroxyproline and uronic acids, which indicates the acceleration of bone resorption in children with osteopenia on the background of the chronic diseases of the gastrointestinal tract. Osteopenia was diagnosed in 59.8% of children with kidney diseases and osteoporosis – in 4.1%. Analysis of the biochemical markers of bone tissue showed imbalanced mineralization of bone tissue: the reduction of ionized calcium and total glycosaminoglycans, increase of daily excretion of hydroxyproline on 30-35% and uronic acids. Revealed changes depend on the duration of illness (r = 0.68), the frequency of exacerbations (r = 0.63) and the severity of renal diseases (r = 0.81). Osteopenia was diagnosed in 51.2% of children with bronchial asthma. Analysis of bone markers showed a significant decrease of ionized calcium on 40%, bone isoenzyme of alkaline phosphatase - on 45% and glycosaminoglycans -on 35-45 units, which confirms the violation of the processes of bone formation. Conclusion: Secondary osteopenia is common for ½ of children with chronic kidney diseases, gastrointestinal diseases and bronchopulmonary pathologies. Pathogenetic aspects of the formation of osteopenia in different chronic somatic diseases in children have certain characteristics that must be considered in their correction.
