ARTERIOVENOUS MALFORMATIONS CLINICAL EXPERIENCE

Background and aims: The problem of treating patients with arteriovenous malformations (AVMs) is still very critical, in spite of modern methods of diagnostics and treatment. Our aim was the estimation of results of surgical treatment of AVMs.
Methods: Analysis based on 44 patients suffering cerebral AVMs who have undergone diagnostics and treatment on the base of the Cerebrovascular center of Kharkov regional hospital from 2008 till 2011. All the patients were examined according to the standards and protocols of medical aid.
Results: Total amount of AVMs - 46. Men - 19 (41,3%), women - 19 (41,3%), children - 8 (17,4%). Division by localization: superficial hemispheric - 36 (78%), deep hemispheric - 4 (9%), posterior cranial fossa - 6 (13%). Multiple AVMs was found at 2 patients. By the source of blood supply: 27 (59%) - superficial branches of ICA, 13 (28%) - branches of vertebro-basilar basin, 6 (13%) - complex from various basins. According to Spetzler - Martin: 8 (17%) - less than 3 cm, 29 (63%) - 3-6 cm, 9 (20%) - more than 6 cm. In 31 (67%) cases Selective Cerebral Angiography was performed. Surgery was done in 27 (59%) cases. Endovascular embolization of AVMs was conducted in 20 cases, other ones was treated by an open removal. The choice of method depended on the patient's condition, localization of AVM, presence of hemorrhage, clinical manifestation.
Conclusions: To improve the results of treating patients with AVMs, the usage of selective specific methods of treatment and diagnostics are needed.
